Instructions for Creating A New Website Account
- Pick a USERNAME that's both easy to remember and easy to type since this will be
your login name. The preferred form to use for USERNAME is: <firstname>.<lastname>
Example: A user named Susan Derth would pick the username: susan.derth
- Make a note of the User Name and Email Address you enter for your account.
If you have trouble this information will help us get you activated as quickly as possible.

are required. Your information will not be shared. This information is required for earning Continuing Education credits.
- A valid, working email address is required.
Email is used for administrative purposes (such as sending password reminders). Your email address will not be shared or publicly displayed on the website.
